Application for registration in a family doctor's patient list

A separate application must be submitted for each family member.
A minor's application must be submitted by a parent or legal guardian. The exception is newborns, who are automatically added to the mother's family doctor list. An application is only required if you wish a different family doctor for the child than the mother's doctor.
