Registration to the practice list

  • Please see our Clinic Rules.
  • The application for registration to the practice list must be submitted via registered mail or email. The application must be manually signed or digitally signed.
  • Separate applications must be submitted for each family member.
  • A minor’s application must be submitted by their parent or guardian.
  • An exception is a newborn, who is automatically added to the practice list of the mother’s family physician. The application must only be completed if the child requires a different family physician.
  • We will notify you by email whether your application was accepted or denied within seven business days of the date of application.
  • We may deny your application if the family physician’s practice list is full or if, according to the population register, the applicant lives outside the practice catchment area.
  • If the practice list is full, we will accept applications only from applicants who have an immediate family member on the practice list or live in our practice catchment area as per the population register.
  • If you are not notified of the denial within seven business days, you will be added to our practice list no later than the first day of the month following your application.
  • Please be sure to provide an extract or a copy of the new patient’s medical record from the previous family physician.
